Trusted Ally created the Safety, Stability, and Longevity (SSL) program to take a more proactive approach to home healthcare by identifying changes in a client's health before those changes become a crisis.

Instead of reactively asking, "What happened?" SSL encourages clinicians to proactively ask, "What’s changed? What risks have emerged? How can we help this individual remain independent longer?”
Health rarely changes overnight and decline usually whispers before it screams. Individually, these gradual changes may not trigger immediate concern, but collectively, they can predict much larger problems ahead.

When these whispers are recognized early, the care team can adjust a Plan of Care, involve physicians sooner, provide education, recommend additional resources, or strengthen support before an avoidable hospitalization or serious event occurs.
SSL creates a structured way for our clinicians to consistently observe, document, and trend these changes over time, identify risks early, intervene before whispers become screams, and allow our clients to thrive.
What whispers may look like: a client walking a little slower than usual. They stop preparing meals. They become less engaged in conversation. They forget medications more frequently. Their home environment becomes slightly more cluttered.
Each component of SSL focuses on a different dimension of healthy aging and together they create a comprehensive picture of the client's overall well-being.

During Safety visits, our clinicians evaluate immediate risks that could result in injury or hospitalization. The focus is to identify today's risks before they become tomorrow's emergency.

Stability visits focus on identifying trends. Rather than looking at one point in time, we can determine whether a client is maintaining, improving, or beginning to decline.
